  • Manager, National Community Partnerships

    • Looking for an enthusiastic professional to join our Boston Financial District-based core team of seasoned healthcare and business professionals in a collaborative, fast-paced, and exciting work environment. 

    • Principal focus is building and nurturing relationships with aging services organizations and other partners in communities that wish to introduce the PACE model of care for their frail elderly residents. Additional responsibilities include:

      • Manage responses to requests for information and/or proposals and PACE applications to state and federal agencies

      • Plan our organization's participation via formal session presentations and informal meetings and strategic conversations at national meetings of aging organizations. 

    • Flexible hours (ability to work from home), while joining the team in the office several days/week. Compensation competitive based on experience.

  • Medical Director, PACE of Northeast Indiana (Fort Wayne)

    • PACE of Northeast Indiana is a partnership between Aging and In-Home Services of Northeast Indiana, Inc. (AIHS), and Edenbridge Health. We seek a mission-oriented Medical Director to join our Program of All-inclusive Care for the Elderly (PACE) program launching in early 2020 to serve the frail elderly of Fort Wayne. The ideal candidate will have the following qualifications, experience, and interest:

      • Geriatric expertise and/or interest and commitment to serving frail elderly population

      • Familiar with or interested in PACE model and principles

      • Comfort and confidence with both outpatient care and inpatient medicine

      • Skilled in managing the risks associated with community-based care of frail older patients without over-reliance on tests, interventions and technology, and in collaborating to evaluate and integrate specialist consultant recommendations within the context of each patient’s unique situation

      • Demonstrated commitment to collaborative, team-based patient care; able to relate to other members of the care team as peers, combining confidence with humility.
